How to obtain the WIRA Water Incident Report Form

Every significant water-related incident should be reported on the WIRA Water Incident Report Form. There are two forms; one strictly for in-water aquatics and the other for transportation incidents.

We recommend you print out the Form along with these instructions for completing a Water Incident Report. Customize the Water Incident Report Form for your organization by filling in any information that will be the same for all incidents within your jurisdiction, before photocopying and distributing the Form. For example, the Agency (name of your organization), nearest city/town, province/territory and county/regional mucipality (depending on the geographic scope of your organization).
